CMS Finalizes CJR-X, Paving Way for Value-Based Surgical Care

The Centers for Medicare & Medicaid Services (CMS) has finalized the Thorough Care for Joint Replacement Expansion (CJR-X) as part of its fiscal year 2027 Inpatient Prospective Payment System (IPPS) rule, marking a significant shift toward value-based care for joint replacements across acute-care hospitals in the U.S.
Effective January 1, 2028, CJR-X will hold nearly all U.S. acute-care hospitals accountable for care related to lower extremity joint replacements during a 90-day post-discharge episode. This initiative aims to reduce healthcare costs while maintaining high-quality care for patients, building on the previous success of the original CJR model, which saved Medicare $112.7 million.
According to CMS projections, CJR-X could generate $725 million in net Medicare savings over its first five years. The initiative is seen as a cornerstone in the broader transition to episode-based payment models, following the earlier implementation of the Transforming Episode Accountability Model (TEAM) last year, which set the groundwork for similar models.
